Gym membership

Can you get a gym membership through Home State Health that pays for it month to month? When I had that insurance they said only the yearly membership fee would be paid and not the month to month bill. I switched over to United Healthcare and they pay for the whole thing. Now I don't really trust UHC & am wondering if I should switch over to HSH & try to get the gym membership, if it will actually cover it all? Does anyone have HSH that was able to get the gym membership?
