High school student recruitment red flags

My partner is a high school teacher. Today, one of her students came in and said the Navy recruiter is giving them a mountain of paperwork. However, the paperwork itself is presenting some red flags.

First, the forms are degraded photocopies. Can they not print original copies?

Second, the title is “Questionnaire for National Security Positions”. This is not an SF-86. Why is a potential enlistee being given a pseudo-SF86? Is this a gimmick?

Third, it says “NRS San Jose”. I assume NRS means Navy Recruiting Station. But San Jose is over an hour’s drive away and our city has its own Navy recruiting station.

Is this par for the course or are these red flags? Thanks!
